Once your District Executive Committee has adopted a proposal for a District / Community Donation, use this form to make the formal request to the AMAPCEO Executive Committee.

Include as much information about the organization, event, or initiative as possible, including how to contact the proposed recipient.
Describe how you believe the request meets AMAPCEO’s objectives.
The AMAPCEO Executive Committee considers requests throughout the calendar year, but be sure to submit your proposal no later than five business days before its October meeting. This is the final meeting of the year where donation requests will be considered.
AMAPCEO staff will review the proposal and may request additional information.
The AMAPCEO Executive Committee accepts proposals on a rolling basis throughout the year. Proposals must be endorsed by your District Executive and submitted at least five business days before the Executive Committee's October meeting for consideration.
AMAPCEO staff will contact your District Director about the Executive Committee’s decision. Your District Executive Committee will liaise directly with the organization to arrange the donation.
