In early 2019 the Board of Directors revised AMAPCEO’s Appointment and Renewal Policy for Health & Safety Representatives and Health, Safety & Wellness Resources.
The revised policy indicates that the Health, Safety and Wellness Committee has the responsibility to review and renew the appointments of H&S Representatives and Health, Safety and Wellness Resources. Each appointment renewal will be for a period of three years.
To conduct its review this fall the Health, Safety and Wellness Committee requires you to answer the questions below as completely as possible by noon on Friday, December 13, 2024. Please note that you are able to complete this form in parts and save an incomplete submission as a draft to complete at a later date.
